Realistic is a difficulty level introduced in Call of Duty: Black Ops III. Realistic is the hardest difficulty level in Call of Duty: Black Ops III. This difficulty can be completed with high skill and patience. Realistic difficulty does not appear in Xbox 360 or PlayStation 3, due to the fact that the last generation version does not have a campaign.
Gameplay
In Realistic, the AI characters are independent, and they will carry weapons with high damage and fire rates, and have greatly changed. The player will also have a very small amount of health which means the player will die by a single shot or explosion.
Completing a level in Realistic difficulty increases the EXP gains by 50%. The player will earn more achievements and trophies for completing the game on Realistic.
Upon completion of a Realistic campaign, the No One Will Believe You and Full Circle achievement/trophies are unlocked.
