It can be in singleplayer or multiplayer. Mine was probably when I was playing The Enemy of my Enemy. I was almost at the UAZ with Price and Rook in it, and broke into a run to get to it. When I was almost there, I was hit and killed by a Shadow Company SUV coming in the other direction.
